It is usually caused by a failure in the automatic defrost system, such as a broken heater or thermostat, which allows ice to block the air vents. A faulty evaporator fan can also prevent the cold air from moving upwards.
Water inside the fridge usually points to a blocked defrost drain. During the normal cooling cycle, condensation forms and drips down the back wall into a small drain hole. If crumbs or food packaging block this hole, the water has nowhere to go and will eventually overflow into the base of the fridge or onto the floor.
To access the compressor, the cooling matrix, or the internal wiring, the fridge freezer will need to be pulled forward.
